I picked up Neverwinter Nights 3 years ago, and I'm still playing it. I probably replayed the official campaigns 5-6 times in row with different classes and I replay some more with the community mods out there. Some of the bigger ones have 100+ hours of gameplay content for just one play-through.

1. What games have you replayed the most number of times?

Burnout 3: Takedown never gets tiring for me, even if the graphics are dated and the AI is simplistic. The later Burnouts don't quite have the same feel.

2.  What games have you played, put down for a few months, and come back to several times?

Nethack

Consoles:
Mortal Kombat and the Tekken series - Used to play against friends all the time.
Final Fantasy Tactics and Tactics Ogre - When solo, and replaying them all again modified with the game shark pro.

PC:
HOMM II+
Civilization III and IV
Homeworld I & II
Galactic Civilizations II

There's actually many popular PC games I've never really tried.  Like I installed Deus Ex, but never got past the tutorial part.

Civ III
Civ IV
Age of Empires 2

Truth be told the vast majority of games I buy I never finish, though I have gotten better lately (mainly through buying less games).

1. What games have you replayed the most number of times?

Lunar: Silver Star Story Complete

Although possibly every other 'Game Arts' developed game that was localized by 'Working Designs' could fall in this category.

2. What games have you played, put down for a few months, and come back to several times?

The 'Half-Life' universe (http://www.youtube.com/watch?v=q1UPMEmCqZo) =D and WoW >_>

^^ That video is just one of the reasons I keep going back to Half-Life.. The saga has so much room for awesome content to add (http://members.shaw.ca/halflifestory/index.htm).

FF VII, FF Tactics (and the PSP version, its even better). Little Big Planet. And that's all I can think of.

If you're into visual novels, Fate/Stay Night.

METAL SLUG 3,4,5.(same as fighting games)
CoD 4, the final Mile High Club mission is the most fun mission around

Let me add Metroid Fusion to that list, which I've played a dozen times. And Descent 2. And just yesterday, I started playing Commander Keen again. How I loved those games, since I couldn't afford a SNES...

Simple one...only 2 answers:
1.RPG's with multiple endings
2.Old games you used to play.

If you can tolerate the graphics and a rather excessive use of figurative speech Planescape:Torment will keep you occupied for quite a while. It has one of the heaviest and intricate dialogue and plot scripts I have ever seen in a video game. (and the game's already 10 years old; work harder, developers!) You'll be playing the game over and over again just to see all the endings, because the game makes you want to. Not even the Final Fantasy or Metal Gear Solid series can come close to it in terms of storytelling.

1. What games have you replayed the most number of times?

Elder Scrolls IV: Oblivion (mainly just to make characters, not so much the game itself)
Final Fantasy IV, V, VI (for SNES and GBA), Tactics Advance
Chrono Trigger (SNES)
Front Mission (SNES), Front Mission 3 (PS1)
WarCraft III (DotA)
Cave Story (Doukutsu Monogatari, indie platformer)

2. What games have you played, put down for a few months, and come back to several times?

Chocobo Racing (still haven't unlocked everyone)
Legend of Mana (forging equipment is a bitch >_<)
Metal Gear Solid
Wild Arms
Pokemon Fire Red/Leaf Green, Pokemon Pinball (Ruby & Sapphire)
C&C Red Alert 2: Yuri's Revenge (I love being sadistic to the AI)
Rollercoaster Tycoon
Theme Hospital
Ragnarok Online
RuneScape (wouldn't go anywhere near it anymore)
O2Jam (using O2Mania, an offline emulator)

And the Tetris in µTorrent!

I would have played TESIII too but it's such an unstable shit even after all these years (the GOTY version still has game-breaking bugs) that I just gave up trying to play it again. I finished it many years ago.
