Here's my two cents on what's going on.
(click to show/hide)I have the feeling that nobody has died, but they're all in comas, and this is the "world" people go to. When they disappear from this world, they are either dying or waking up from their coma.

But something like that did cross my mind...
At the very least, I'm certain (click to show/hide)we're going to see those who disappear again. Where they go is still in speculation with me though. But if I had to guess, I'd say it's a place better than where they're at now, and at the end Yuri is going to be alone in this world angry that everyone's left/abandoned her. Then she's gonna end up going to the world where everyone's happy and no longer be alone.
We shall wait and see!

They said so far that they are in the place that people who had terrible things happen in life or just crummy lives end up. My best guess is the obvious they cant let go of their past cant move on form the things that scarred them. Once they come to terms with their past they get reborn most likely into better lives than their previous life. There may be a twist or two on the way but thats largely how i see it fleshing out.
