Seriously, the social taboo against it is *SO* strong in most places, that it would be pretty rare for it to happen.
I'm NOT saying that it doesn't happen, most cases are probably 'one offs' or single instances.
The chances of there being cases of 'ongoing relationships' of that sort would be very small indeed.
As far as rape goes.... I don't go there.
If the other person involved doesn't want to do it and you go ahead, it's rape, even if you are currently married to said person.

Cousins don't particularly possess similar enough genetic material to be much of a concern. It's common practice in many cultures (and even desirable) for cousins to marry. [1] The further removed, obviously, the safer. First cousins are only about 2-3% more likely to pass on disease than a totally non-related couple, so you're pretty safe. If that's what floats your boat, go for it! ;)
