All in all, one of the best episodes yet. Hoping they keep it up like this through the end now ...
Mei knows who it is,
but she's been keeping quiet? Or, maybe her vision tells her who is contaminated with death .. so she sees it on several people without necessarily knowing which one is the Extra. Like Cassandra, she just can know who is doomed beforehand without being able to do anything about it.
Why would the glass eye that her "mother" gave her have such a power? Twin daughters of a twin sister, what weird connections there?

I think we can call this now. Misaki Mei might be lying about what her eye sees and the other Misaki actually being her sister, but assuming she isn't then Head of Countermeasures, Akazawa Izumi is the extra student. She and Sakakibara Kouichi have met before, in Yomiyawa. The phenomenon is obscuring this by altering memories. That strongly suggests one of them is dead. It's not Kouichi. Therefore, it's Izumi.
I think this is what happened:
* Kouichi was in Yomiyawa around 2 years ago.
* Kouichi and Izumi met during that time.
* Izumi's brother was in Class 3 that year.
* Izumi dies due to the phenomenon.
* Izumi comes back in April as the extra.
I'm not sure what happened to Izumi's brother though. How did he make Izumi cry? Did he just die as well? That seems a bit too simple.
I'll credit you with about as good a guess as I've got at the moment. I've already been proven wrong on several of mine ...
One thing jumps out, though ...
Izumi remembered Kouichi in a dream. Her reaction in Ep. 01 when she touched his hand now comes full circle; there has been a "ghost" of a memory playing with her. But, if she were dead, would she really dream? I'm thinking her memories are definitely being fucked with, but not because she's the Extra. Interesting reveal that dreams might see Truth and be able to bypass at least some of the altered memories.
This may also now be corroborating evidence that Kouichi did live in Yomiyama two years ago, as his father commented 'accidentally' in a previous episode. Izumi could not have been dead when she met Kouichi. Hmmm. Although, her meeting him (before or after her death, if that happened) could be a reason for her ghost to return as the Extra; but why would she have become part of Class 3 before Kouichi's transfer? Would be mind-bending creepy if she was dead when she met Kouichi in that dream sequence and Kouichi was able to see & interact with her then.
On 'another' note, though: if Izumi's brother was previously a casualty of the Class 3 phenomena ... it really sucks that information was not brought up before now. It seems clear that is a very real possibility ... but, are memories so completely screwed with that people don't remember their own missing siblings from previous cycles? Only two years ago? NOBODY in the story remembers that ..?
Makes me be thinking that Izumi's brother may be the Extra in this years' Class 3. Just need to ID him. Izumi didn't die before, her brother did, and Teshigawara is Izumi's brother, now the Extra? (Or Mochizuki). But, Izumi does seem to have it in for Teshigawara ... an old, unresolved sibling issue affecting them unconsciously?
And, Teshigawara seems to have an unhealthy obsession with who the Extra is, perhaps even causing more death ... maybe he is already beginning to suspect he might be the Extra and is trying desperately not to realize it?
Izumi made some irrational/incorrect statements in her little speech @~10:30, though.
Koichi met Mei in the hospital before anyone knew they were classmates, and Teshigawara bears at least as much "responsibility" (by Izumi's logic) by his behavior later, but didn't get called out.
Last class trip/visit there were 19 students plus one extra ... the group photo this time demonstrates 18 students, plus #19 taking the photo, plus two teachers ... one who is "supposed" to be immune to the curse.

how can she be so sure that she is not in danger? Very odd comment @~12:50. Or, maybe she doesn't care about herself at this point and just meant others are more worth worrying about than herself.
Mostly, what I hope to see is them trying to resolve the Phenomena permanently, not just trying to stop it for this cycle only. My previous comments about Izumi seeming to be unusually dedicated to her position as Head of Countermeasures ... I think she has the pieces to put it together and the will power to try and do whatever it takes to try and stop it permanently.
I still like the idea that Kouichi's mom is the Extra this time. Perhaps even as Izumi. Maybe Izumi never died because she was never alive ... she's "just" an Extra for someone else who was once alive. There is no assurance, either, that there is always only one Extra; what if there were two? Sometimes?
The question asked was "Could you kill your own classmate?" But, I think the more important question is "Could you kill yourself?"
