actually matching your table's height to your elbow's should make the wrist straight, an adjustable seat solves the problem.

My elbow actually rests on the desk. This takes the weight off my shoulder and elbow. My mouse is fairly bulgy. I have considered getting a flat mouse like the apple mice (but with more than one button and not over priced).
