Now and Then, Here and There.

More disturbing than any other anime I've seen, because it doesn't just use a grimdark setting or themes as some. Now and Then, Here and There genuinely tries to present a world distilling all the fucked up things humans have done and are doing now until you want to cry in despair. It's worse than hentai sadism, because none of that is in earnest, mere wish fulfillment for the id. This is like Grave of the Fireflies without the humanity.

Every atrocity you'd rather not think about to keep yourself happy and sane, this has it.

Elfen Lied tends to be the most gruesome shit you've seen when you watch it for the first time, but then you look back on it after watching more shows and laugh at the over-exaggeration of most of the scenes. Or maybe that's just me.
