The point in Kashimashi is that Hazumo isnt a transexual though, he changed sexes but he actually became a girl, not just a 'cut and tuck' operation like one of todays 'sex change' operations. Everything works, no hormones needed, pregnancy possible etc etc.

Only in Japan you'd find a story like it though.
